DUTIES: The Case Manager's primary responsibility is to provide case management services to individuals with mental illnesses in agency, homes, hospital, schools and other appropriate community settings. Case management services typically consist of assisting clients to access resources including financial aid, housing, food, medication, etc. Services also include assisting with teaching skills of daily living, assuring access to medical/psychiatric services, providing needed transportation, supporting/educating caregivers, and completing clinical paperwork including treatment plans, progress notes, quarterly case reviews, and discharge plans. The case manager also will provide inter-agency consultation/technical assistance and community education.

MINIMUM QUALIFICATIONS: Prefer applicant have a Bachelor's Degree in Psychology, Social Work or a related field with 1 year experience working with individuals with mental illness or an Associates of Arts Degree and two years' experience with working with individuals with a mental illness. Good written & oral communication skills and be able to complete administrative & clinical records. Computer literacy required - clinical software used. Travel required within the Steven's County. Must have driver's license, vehicle insurance.

BENEFITS: National Health Service Corps Loan Repayment Program. Annual salary $53,720.16-$72,112.92 based on qualifications and experience.
